📝 CAN-19 Signature Collection Instructions
For Volunteers Supporting Floyd Michael Taylor, Independent Candidate for U.S. House (IN-9)
✅ What You’re Collecting
- Valid signatures from registered Indiana voters
- Goal: 4,500 verified signatures (we’re aiming for 6,000+ to be safe)
- Signers can live anywhere in Indiana—not just District 9
📄 What You’re Using
- CAN-19 petition form (2026 version) — printed double-sided
- Front (Page 1): Signature sheet
- Back (Page 2): Circulator affidavit
- Top section is pre-filled with:
- Candidate Name: Floyd Michael Taylor
- Address: 99 E US Highway 150, Hardinsburg, IN
- Office Sought: U.S. Representative, District 9
- You only need to fill in the county at the top of Page 1
🖊️ How to Collect Signatures
- Use one sheet per county
- Write the county name clearly at the top of Page 1
- If a signer is from a different county than the one listed, start a new sheet for their county
- Ask each signer to complete all fields:
- Printed name
- Signature
- Residence address (not mailing address)
- City or town
- County
- Date of signing
- Do NOT pre-fill or alter any voter info
- After collecting signatures, complete Page 2:
- Fill in your name and address
- Sign and date the affidavit
- Confirm you witnessed each signature
🚫 Common Mistakes to Avoid
- Mixing counties on a single sheet
- Forgetting to sign the circulator affidavit
- Using outdated forms (only use the 2026 version)
- Accepting signatures from unregistered voters
- Leaving fields blank or illegible
📍 What to Do When You’re Done
- Return completed sheets to campaign HQ:

- Or contact Floyd directly to arrange pickup/drop-off
- Do not submit sheets to county offices yourself — campaign will handle routing and verification
